Access & Triage Co-Ordinator

Job summary

We have an exciting vacancy within the Access & Triage Team at Tanworth Lane for an Access & Triage Coordinator to join our team. You will be an integral part of the triage team, providing a professional and effective response to the patients who access our services. This will be both online and over the telephone sign posting patients correctly to ensure they receive the right care, at the right time by the right healthcare professional for their individual care needs. You will provide support to the clinical triage team, designing and monitoring rotas, managing the appointment system and providing support and training for the administrative staff.

Main duties of the job

The post holder will deliver a service in a proactive and engaging manner following the guidelines and protocols of triage.

  • Probe and question patients effectively and document patient information with a high level of accuracy and appropriateness.
  • Managing and monitoring the triage rotas and appointment system.
  • Processing both online and telephone requests for appointments, visits and telephone consultations ensuring patients are directed to the appropriate healthcare professional.
  • Initiating contact with and responding to requests from patients, other team members and associated agencies and providers.
  • Promote, advise and assist patients accessing services via online portals.
  • Work closely with the triage clinicians, offering administration support via inhouse systems.

Tanworth Lane Surgery is part of a wider group of GP Practices working together.

Flexibility to work at the other premises when requested is required as part of this role.

The locations of the other key GP Practices are listed below but are not exclusive. Other locations may be added to this list.
