Job summary

The post-holder will ensure that GPS Healthcare is maximising our processes around medicines management and access, are responding to our patients needs, and are maximising our contractual requirements. The postholder will support the chief pharmacist in the development and implementation of workstreams. They postholder be skilled in audit and quality improvement as well as being able to manage patient related clinical work.

As the senior PCN pharmacist, the postholder is expected to contribute to the leadership of medicines related issues for the PCN, and to provide leadership and mentoring to any PCN clinical pharmacists, PCN pharmacy technicians and prescribing support staff employed across the PCN.

Main duties of the job

  • To reduce the medicines related workload on doctors.
  • Provide leadership and hands-on involvement, to ensure the practice is compliant with CQC standards where medicines are involved.
  • Increase safety and quality of prescribed medicines.
  • Provide support and be a point of contact on medicines related issues to the whole practice team and patients.
  • Undertake clinical medication reviews with patients with multimorbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
  • Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review).
  • Medicines reconciliation on transfer between care providers.
  • Care home/Frailty medication reviews.
  • Managing caseload of patients with common/minor/self-limiting ailments while working within scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
  • Support the Acute Care Team and Triage team in managing medicines related urgent queries.

Leadership Role

Provide leadership in supporting further integration of general practice with wider healthcare systems.

Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.

Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.

To support the chief pharmacist in delivering on KPIs and strategic organisational goals.

Demonstrate understanding of, and contribute to, the workplace vision.

As part of the medicines management team, to monitor practice prescribing against the local health economys RAG list for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Liaise directly with hospital colleagues where prescribing needs to be returned to specialists.

Lead and provide support to other clinical pharmacists, pharmacy technicians and prescribing support staff.

Line management and leadership of PCN pharmacists and technicians, objectives, ensuring all team members have clarity of purpose.

Support educational development of PCN clinical pharmacists and future pharmacists/independent prescribers in the form of clinical supervision and mentorship.

Understand the implications of national priorities for the team and/or service and manage the team

through these changes.

Identify and resolve risk management issues according to policy/protocol.

Practice & Service Development Work

Focusing on QOF Domains, improving disease registers.

Act as a source of medicines information for all of the practice team and patients, (such as around doses, side effects, adverse events, possible alternatives, i.e. around out of stocks). Suggesting and recommending solutions.

Prescribing Safety audits, Clinical audit, Local Enhanced Services support.

Lead on prescription management (systems and processes, ensuring monitoring is being done, support clerical staff).

Actioning of MHRA safety alerts and lead on clinical safety team meeting as required by the Chief Pharmacist.

Provide guidance and/or training on important prescribing messages to improve prescribers knowledge.

Participate in the PCN MDT.

Liaise with colleagues including ICB,STP/ICS Pharmacists and Pharmacy Technicians on prescribing related matters to ensure consistency of patient care and benefit.

Liaise with other stakeholders as needed for the collective benefit of patients.

Work with the Chief Pharmacist and clinical leads to develop and manage new services that are built around new medicines or NICE guidance, where new medicine/recommendations allow the development of a new care pathway (e.g. new oral anticoagulants for stroke prevention in atrial fibrillation).
