Amersham Health Centre are looking to recruit a Home Visiting Paramedic to join the team. The role is essential in providing the best quality patient care to patients based in their own home or in a residential care home with on-the-day urgent/acute visit request and essential annual reviews.
The post holder will be an experience Paramedic, who will support and collaboratively with the Practice Team to carry out home visits that operate during the working day. The successful applicant will have the experience, knowledge and skills to provide holistic care for individuals including assessment, diagnosis, treatment and management.
Our Integrated Care Board are supporting are supporting Paramedics with Primary care to develop skills and knowledge in line with Health Education England - First Contact Practitioners and Advance Practitioners in Primary Care (Paramedic): A Roadmap to Practice.
The post holder will be a valued member of the team and lead the home visiting function to support the work pressures that are evident within GP Practices.
Main duties of the job
KEYRESPONSIBILITIES
- Work as an autonomous and accountable paramedic.
- Rapidly assess and treat patients in various acute medical emergencies.
- Administer appropriate medical interventions and medication under established protocols.
- Collaborate with doctors, nurses and other healthcare providers to ensure seamless patient care.
- Maintain accurate and thorough patient records.
- Educate patients and their families about medical conditions, treatment plans, and preventive measures.

- Collaboration and Communication: Work closely with GP's, nurses, social prescribers and other healthcare providers to ensure coordinated and comprehensive care for patients and to communicate the patient's status with the relevant team in the surgery.
- Patient Advocacy: To advocate for the patients needs and to ensure they have access to appropriate services and resources.
- Palliative and End-of-Life Care: Provide palliative and end-of-life care, offering comfort and support to patients and their families during challenging times.
