Primary Care Network Care Coordinator
An exciting opportunity has arisen within Primary Care to work as a Care Coordinator in our Churchill, Canbury, Orchard and Berrylands Primary Care Network (CCOB PCN). We care for approximately 50,000 patients in four practices across the borough of Kingston.
You will need a caring and compassionate nature, excellent organisational skills and to enjoy working with people. The world of General Practice is busy and ever changing so an ability to learn quickly and be adaptable to rapid change is essential. The role will require an ability to organise and prioritise your own workload, and so you should be comfortable working independently and as a committed member of a multi-disciplinary team.
The role is pivotal in ensuring patients can access care both within primary care and secondary care or community services.
Main duties of the job
The main duties of the role will include
Provide a practice point of contact for patients in coordinating their care
Keep patients medical records up to date through filing of letters and emails
Ensure timely follow-up and action for patients from communications from community or secondary care
Deal with incoming queries from patients, staff and other healthcare providers
Organise in-practice appointments, follow ups or other actions to help the PCN practices provide high quality, compassionate care to our patient population
The role will also involve working closely with other healthcare professionals across the PCN and local health economy to ensure patients receive high quality care. You will need to be educated to at least GCSE level or equivalent and have experience of working with patients. Experience of working in a healthcare setting would be desirable, and a working knowledge of primary care helpful. You will also need good computer literacy.

Sort incoming post and allocate to clinical or non-clinical workstreams as necessary
File and task clinical documents in line with practice procedures
Deal with incoming practice email queries
Act on communications from hospitals or community providers
Assist First Contact Reception Team with queries relating to administrative duties
Answer administrative anima queries from patients
Review any new cancer diagnosis patients and consult with our in-house social prescriber for cancer care review
Manage incoming SAR requests (medical record requests) from patients and third parties
Manage requests for private work -insurance reports, TWIMC letters etc in line with practice policies
Monitor EMIS tasks to ensure they are completed and care delivered through regular audit of the clinical system
Run regular system audits to ensure follow ups arranged for patients
Organise and prioritise own workload
Respect and maintain privacy and dignity of patients; assure patient confidentiality at all times by documenting/adhering to patient consent requests
Support patients and carers; collect feedback on their experiences
Support the PCN practices to deliver on QOF, KPIs and locally commissioned enhanced services
Participate in practice meetings, patient participation groups and other meetings to improve engagement with the role
