Main duties of the job

Job responsibilities
Provide high-quality nursing care to patients within our
practice.
Assist in the management of chronic diseases, with a
particular focus on Diabetes.
Engage in health promotion and education for patients to
enhance their well-being.
Collaborate with a multidisciplinary team to deliver
patient-centred care.
Conduct health assessments, screenings, and vaccinations as
required.
Assess, treat and advise patients
presenting with nursing needs.
Ensure clinical practice is safe and
effective and remains within boundaries of competence and to acknowledge
limitations.
Perform investigatory procedures,
including collection of pathology samples.
Assist GPs / Nurse Practitioners with
minor procedures.
Work within the clinical/nursing
policies, procedures and protocols.
Ensure any patient information is
up-to-date and produced in appropriate formats.
Undertake new patient and routine
assessments of patients.
In general the post-holder will be
expected to undertake all the normal duties and responsibilities associated
with a Practice Nurse working within primary care.
