An exciting opportunity has arisen for an enthusiastic Health Care Assistant to join our supportive team. Havelock Grange Practice are looking for a experienced Health Care Assistant who can support the Practice Nursing Team.
The successful candidate will deliver safe, high-quality primary care services to our patients and assist the nursing and clinical team in the provision of treatment, preventative care, health promotion and patient education. Working as part of the Practice multidisciplinary team, delivering care within their scope of practice to our patients.
Main duties of the job
The Health Care Assistant will work
under the supervision of a registered clinician while developing competencies
in community-based care.
The post-holder will be an integral
part of the general practice team.
The
following are the core responsibilities of the Health Care Assistant in
delivering health services:
- Consult
with patients and their families to undertake assessments of patient need
- Deliver and assist clinical staff in the provision of
treatment, preventative care, health promotion, long-term condition management
and patient education
- Provide
clinical support to registered nurses and other healthcare professionals within
the organisation
- Deliver
basic clinical care interventions under supervision, supporting continuity of
care and patient safety initiatives
- Act
as a chaperone as required
- Ensure
specimens are recorded and ready for onward transportation
- Provide
support during minor operations as required
- Support
in the delivery of enhanced services and other service requirements

centred approach.
We are an experienced training
Practice with 6 GP Partners, supported by a diverse team of Advanced Nurse
Practitioners, Nurses, Health Care Assistant, Phlebotomists and resilient
Reception and Admin teams.
We also work as part of the One Life
Hartlepool Primary Care Network, which has three GP Practices working in
partnership; This allows the Practice to be also supported by PCN Pharmacists,
Mental Health Practitioner, First Contact Physiotherapy, Social Prescribers,
Care Co-ordinators and Health & Wellbeing Coaches.

occasion, a requirement to carry out other tasks. This will be dependent upon
factors such as workload and staffing levels:
