Job summary
We are looking to recruit an Advanced Practitioner to
work in our busy practice serving 22,300 patients across three sites; this post
will generally cover our two main sites.
Requirements of the post are previous experience of working
in a Primary Care type setting, being able to independently prescribe and
receive patients with undifferentiated and undiagnosed problems and make an
assessment of their health care needs.
Main duties of the job
The role will involve a mix of providing care in a surgery setting, seeing patients in booked appointments at ten-minute intervals and patients own homes, working through a patient list, treating minor illnesses and prescribing as necessary.

Wychbury Medical Group is an established practice providing care for over 22,300 patients from 3 Dudley registered surgery sites. The organisation currently has 19 GPs, 7 of whom are Partners. The practice has a diverse population and has surgery sites situated in varied residential locations within Dudley. The local area is within close reach of wonderful countryside and also provides excellent access to Birmingham city centre; there is a good transport infrastructure with easy connection to UK motorways and excellent local schools.

Diagnosing and treating patients presenting with minor illness
- Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
- Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary
Duties and Responsibilities of Post
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients in order to support compliance with, and adherence to, prescribedtreatments.
- Provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactions
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
- Recognise and work within own competence and professional code of conduct.
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
