Job summary

We are looking for a qualified Nursing associate who would be willing to upskill to assist with our practice nurses in managing our patients with chronic diseases. This would include upskilling to be able to manage chronic kidney disease, hypertension and respiratory related diseases.

We are a large forward thinking practice and we are willing to invest in the right applicant.

Main duties of the job

The post holder is a Nursing Associate registered with the Nursing and Midwifery council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within General Practice.

As a Nursing Associate, you will work under the supervision of and in partnership with the Nursing team to provide and deliver a high standard of nursing care. You will contribute to the on-going assessment, providing and monitoring care based on evidence and working in partnership with the multi-discplinary team.

Knowledge, Training and experience

Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.

Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition, be able to identify anomalies that should be referred to a first level nurse or one of the medical staff Baseline observations include:

Elimination pattern

Glucose monitoring

Demonstrate an ability to complete ongoing monitoring templates
