Job summary

The successful candidate will be a qualified Paramedic with significant experience in the assessment and management of patients presenting with acute medical needs, ideally gained within a primary care or urgent care setting. However, candidates with extensive frontline paramedic experience will also be considered.

The post-holder will work as part of a well-established and expanding multidisciplinary team (MDT). This team includes paramedic colleagues, General Practitioners (GPs), Advanced Clinical Practitioners (ACPs), nurses, physiotherapists, Healthcare Assistants (HCAs), and other healthcare professionals working collaboratively to provide high-quality patient care.

The Paramedic will be expected to integrate into the team, make autonomous clinical decisions within their scope of practice, and support the delivery of safe, effective, and patient-centred care across the practice population.

The role will initially involve in-clinic working, with the potential for future expansion into a mixed model of clinic-based and community-based care as the service develops.

The ideal candidate will be supported and will work within an ever-growing team of practitioners at varying stages of the advanced practice journey. The supervision and mentorship network will be provided to support the Paramedic.

Job description

Job responsibilities

The following are the core responsibilities of the Paramedic delivering health services.

a. Work as part of a multi-disciplinary team within the organisation

b. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment, including prescribing medications, following policy, patient group directives, NICE and local clinical guidelines and local care pathways or make necessary referrals to other members of the primary care team

c. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the social prescribing service and, where appropriate, other community or voluntary services

d. Be able to

Perform specialist health checks and reviews within their scope ofpractice and in line with local and national guidance

Perform and interpret ECGs

To order necessary investigations and provide treatment and care both individually and as part of the primary care team and through referral to other agencies.

e. Support the delivery of anticipatory care plans and lead certain services e.g. care home

f. Represent the Care Home team at the MDT.

g. Provide an alternative model to urgent and same day GP home visit for the network

h. Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered

i. Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required

j. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient healthcare and registration with the organisation

k. Communicate effectively with patients and, when appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices

l. Receive referrals and directed patients from triage services and other clinicians

m. Maintainaccurateclinical records in conjunction with extant legislation

n. Develop yourself andtherole through participation in clinical supervision,trainingand service redesign activities

o. Actively signpost patients to the correct healthcare professional

p. Review the latest guidance, ensuring theorganisationconforms to NICE, CQC etc.

q. Be aware of duties and responsibilitiesregardingcurrent legislation and adhere topracticepolicies and procedures on safeguardingadultsand safeguardingchildren

r. Support in the delivery of enhanced services and other service requirements on behalf of theorganisation

s. Participate in the management of patient complaints when requested to do so, andparticipatein the identification of any necessary learning brought about through clinical incidents and near-miss events

t. Undertake all mandatory training and inductionprogrammes

u. Contribute to and embrace the spectrum of clinical governance

v. Attend a formal appraisal with your manager at least every 12 months. Once a performance/trainingobjectivehas been set, progress will be reviewed on a regular basis so that newobjectivescan be agreed

w. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care

x. Maintain a clean, tidy, effective working area at all time

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.
