Clifton Medical Centre are looking to recruit an enthusiastic and proactive practice nurse with experience in Minor Illness, to join our town centre practice.
Located in the centre of Rotherham, and with a branch surgery at Wickersley, we serve a varied population of approx 13,400 patients.

Main duties of the job
- Run autonomous minor illness clinic
- Identify, assess and diagnose patients with complex health needs across the Practice population
- Prescribe products to meet the needs of patients within the Practice population
- Support development of programmes to improve health and well-being and meet the specific needs of communities, groups and individuals
- Work with others to change policies to improve health and well-being and reduce inequalities
- Support the implementation and evaluation of evidence-based practice to improve the services offered by the Practice nursing team
- Support the team in quality assurance, development and clinical governance.
- Maintain all associated records, both clinical and manual.

- Assessment, Diagnosis, and Treatment
- Independently assess, diagnose, and treat patients presenting with a range of minor illnesses, following national and local guidelines.
- Prescribe medications as appropriate (if qualified as an Independent Prescriber) or liaise with prescribers in the team.
- Identify and manage long-term conditions where appropriate and escalate concerns to GPs or other clinicians as needed.
- Clinical Decision-Making
- Use advanced clinical skills and evidence-based practice to support decision-making.
- Recognise signs of serious illness and refer patients appropriately for further assessment, investigation, or secondary care.
- Patient Education and Health Promotion
- Provide health education and advice on self-care, lifestyle changes, and appropriate use of NHS services.
- Promote healthy living and disease prevention in the local community.
- Documentation and Record-Keeping
- Maintain accurate, timely, and comprehensive patient records in line with practice protocols and confidentiality standards.
- Teamwork and Collaboration
- Work closely with GPs, other nurses, pharmacists, and allied health professionals to deliver joined-up care.
- Participate in team meetings, audits, and clinical governance activities to maintain and improve service quality.
