Job summary
We are seeking a proactive and organised Pharmacy Technician
to join our dedicated team within a busy surgery. This role offers an exciting
opportunity to contribute to the delivery of safe, effective and high quality
patient care through medicines optimisation and prescribing support.
Working closely with GPs, clinical pharmacists and other
healthcare professionals, you will assist with medicines reconciliation, repeat
prescribing and prescription queries, while promoting cost effective and
evidence based use of medicines. Youll also play an active role in supporting
audits, medication reviews and patient education initiatives. The ideal candidate will be a registered
Pharmacy Technician with the General Pharmaceutical Council (GPhC), with a good
understanding of medicines management and strong communication and IT skills.
Experience in primary care is desirable but not essential, as full training and
support will be provided. This is an
excellent opportunity for a motivated pharmacy professional to develop their
skills within primary care and help improve the quality and safety of
prescribing for our patients.
Main duties of the job
The role involves leading medical review processes by reviewing hospital discharge summaries, outpatient letters and care home medication lists, as well as preparing for Structured Medication Reviews (SMRs) in collaboration with clinical pharmacists. It includes providing guidance to patients on the safe use of medicines, supporting adherence and responding to medication related queries. The position also entails running searches, generating prescribing reports, and supporting QOF, Ardens, IIF and DES targets, while assisting with local medicines optimisation audits. Active participation in practice and Primary Care Network (PCN) meetings, case discussions and medicines optimisation planning is essential to ensure effective multidisciplinary collaboration. The role requires commitment to professional development through continuous learning, mandatory training, and pharmacy service improvement. Additionally, it supports PCN-wide initiatives focused on formulary compliance, medicines safety and integrated care projects. Maintaining accurate patient records, documenting interventions and updating protocols or standard operating procedures (SOPs) form a key part of the administrative responsibilities.
