Main duties of the job
In general, the post-holder will be expected to undertake
all the normal duties and responsibilities associated with a GP working within
primary care.
The provision of services commensurate with the GMS contract
Making professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred from other health care
workers within the organisation
Effective management of long-term conditions
Prescribing in accordance with the practice prescribing
formulary (or generically) whenever this is clinically appropriate
Processing of administration in a timely manner, including
referrals, repeat prescription requests and other associated administrative
tasks
On a rotational basis, undertake duty doctor provision
Maintain accurate clinical records in conjunction with good
practice, policy and guidance
Working collaboratively, accepting an equal share of the
practice workload
Accept delegated responsibility for a specific area (or
areas) or the QOF
Adhere to best practice recommended through clinical
guidelines and the audit process
Contribute to the successful implementation of continuous
improvement and quality initiatives within the practice
Ensure compliance with the appraisal process & prepare
and complete the revalidation process
Support the training of medical students from all clinical
disciplines
Support the partners in achieving the strategic aims of the
practice, making recommendations to enhance income and reduce expenditure
About us
Park Medical Centre is a successful, growing GP Training Practice
in the thriving community of Hoole, Chester. with a population of 10,500
patients. We are a member of the forward-thinking Chester East PCN which is
made up of four collaborating practices (Boughton Health Centre, Heath Lane
Medical Centre and Upton Village Surgery), covering a population of around
37,000, successfully delivering a range of clinical services. Additionally, we
are a member of Primary Care Cheshire (Federation) which primarily provides
support services for Enhanced Access.
You will be working with an experienced operational
management and administrative team, along with a highly skilled clinical team
including 5 GP Partners, 2 Salaried GPs, Nursing Team (3 ANP, 1 Practice Nurse,
Nurse Associate, 1 HCA and 2 GPAs), 2 Clinical Pharmacists, First Contact
Physiotherapist, and a Mental Health Practitioner. We pride ourselves on a
supportive environment, evidenced by an open-door policy and daily practice
huddle.
GP appointments are 15 minutes with up to 12-14 contacts a
session, and are a mixture of face to face, telephone and follow up. We are
generally a low visiting practice, with no CQC registered care homes to
service.
We use EMIS Web, Docman, PATCHS and AccuRx as our primary
systems for patient management and communication.

The post holder will work as one of a team of GPs and nurses delivering general practice services to non-registered and registered patients at Park Medical Centre (PMC).
Clinical Responsibilities
In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties as required, including:
- Surgery consultations Consultations will cover the breadth of the Practices core GMS and Enhanced Service obligations, which you will be expected to support the Partners in delivering
- Additional or emergency consultations
- Telephone consultations and queries
- Visiting patients at home or at care providers premises
- Share the daily responsibility of checking and signing prescription requests and dealing with queries
- Reviewing and following up results from requested tests, scans, investigations and referrals in a timely fashion
- Clinical paperwork and follow up as allocated via PMCs chosen workflow management system in a timely fashion
- Providing clinical supervision to the nursing team and support to the management team and administrative staff as appropriate
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health
- Overseeing and participating in chronic disease management
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat prescriptions, increasing uptake of the Electronic Prescribing Service (EPS) where in the patients best interest
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate, participating in reducing PMCs prescribing spend as appropriate

- Assisting the practice in maintaining GP training status
