Job summary

This is an exciting opportunity to join a forward thinking Practice, providing the highest standard of patient-centred health care.

We have a population of up to 10,000 patients and we lead the way for Primary Care in Huddersfield. Our Practice's QOF figures are consistently high and we provide scope for our Clinicians to develop special interests in Primary Care.

Main duties of the job

The successful candidate will be responsible for managing patients with long term conditions as part of a large, experienced clinical team including GPs, ACPs, Practice Nurses, Pharmacists and Health Care Assistants. We will welcome full time or part time applicants to be considered for this opportunity.

Salary will be negotiable, depending on experience.

About us

We are a supportive training practice with 5 GP partners, Salaried GP, ACP and 2 GP Registrars. We have excellent supporting administrative and clinical teams including Practice Nurses, Healthcare Assistants and Pharmacy team. We have approximately 9,700 patients and operate from purpose built premises. We are a member of the Tolson Care Partnership Network. We use TPP SystmOne and are high QOF achievers with a Good CQC rating.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations, sexual health services and well-person check-ups.
