Job summary

The North Liverpool Primary Care Network would like to recruit an Advanced Nurse Practitioner to complement our current Care Home Team.

Main duties of the job

We are now seeking a dynamic and experienced Frailty Advanced Nurse Practitioner (ANP) to join our growing network team to lead on the delivery of advanced frailty care within our care home population.

Job description

Job responsibilities

The postholder will:

  • Provideclinical assessment, diagnosis, and management of residents living with frailty within care homes across the network.
  • Work alongside the patients registered GP to support the delivery ofanticipatory care planning, including advance care and end-of-life discussions.
  • Manage acute presentations and long-term conditions, ensuring timely, evidence-based interventions.
  • Work closely with GPs, pharmacists, therapists, and community teams to deliver holistic, coordinated care.
  • Participate in and contribute to multidisciplinary team (MDT) meetings and care reviews.
  • Support structured medication reviews and contribute to deprescribing initiatives.
  • Provide clinical leadership and education to care home staff to enhance clinical confidence and competence.
  • Contribute to implementation of the Enhanced Health in Care Homes (EHCH) framework and the PCN frailty strategy.
  • Participate in audit, quality improvement, and service development projects.

Person Specification

Qualifications

Essential

  • Registered Nurse with current NMC registration
  • MSc in Advanced Clinical Practice (or equivalent)
  • Fulfil the Advanced nurse practitioner specifications as per PCN DES
  • Independent Non-Medical Prescriber (V300)
  • Vaccination awareness within primary care

Desirable

  • Knowledge of frailty assessment tools
  • Experience in service development, leadership, or teach
  • EMIS user skills
  • Ability to develop and implement policy and procedure to sustain a quality service

Experience

Essential

  • Experience working in Primary Care, Community Nursing, or Care Home Liaison roles.
  • Experience of working within a nursing teams
  • Experience of dealing with vulnerable patients
  • Proven ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication, clinical reasoning, and decision-making skills.
  • Full UK Drivers Licence and transport
  • Understanding of DNAR-ACP
  • Understanding of comprehensive geriatric assessment toolkit
  • Understanding of structured medication reviews
