Job summary
To
work as part of the multidisciplinary team, to provide advice and support on
all prescribing issues. When clinically appropriate, to implement and action
changes to the patients medical prescribing record as directed by primary,
secondary or community care; working at all times within professional limits.

Main duties of the job
To
work as part of the multidisciplinary team, to provide advice and support on
all prescribing issues. When clinically appropriate, to implement and action

Liaising with the Medicines Management Team,
having at least quarterly meetings to review progression
Liaising
and working with PCN Care Home team
Liaising
with Practice allocated PCN Pharmacy Team
Completing
Care Home SMRs; pre work/visits/post work every 6-12 months.
To
proactively manage patients with complex polypharmacy and care home residents
Providing Annual Leave cover for Pharmacists PCN
wide at a rate of 50% of time
To
influence the prescribing behaviour of all members of the Practice MDT
including GPs and practice nurses.
Liaise
with health professionals in secondary care on interface medicines management
issues.
Carry out medication reviews for every new care home
resident within 4-8 weeks
Ensure
prescribing and administration of medicines and disposal of medicinal waste
follows appropriate legislation and health and safety policies.
