About the Role As part of our clinical pharmacy team, youll help improve the quality and safety of prescribing and support medicines optimisation across the practice. Youll play a key role in medicines reconciliation, patient support, audit work, and reducing prescribing workload for GPs

- Support safe, effective prescribing and medicines optimisation
- Provide advice to clinicians and patients on medicines queries
- Carry out medicines reconciliation and clinical audits
- Contribute to prescribing safety and local enhanced services work
- Liaise with community, hospital, and ICB pharmacy teams
