Job summary
This role helps people manage longterm health conditions by offering safe, personcentred clinical care that supports their independence and wellbeing.
Youll work with our friendly nursing and clinical teams to assess, treat and advise patients in a general practice setting. Your work will focus on conditions such as asthma, diabetes, chronic obstructive pulmonary disease and high blood pressure. Youll offer care in person, by phone and through digital platforms. Contracted hours will be worked between Monday and Friday, 8am to 6.30pm.
What youll do:
Carry out clinical assessments and support people with longterm conditions
Provide inperson, telephone and digital consultations
Manage undifferentiated illness and acute presentations
Use advanced clinical skills, including independent prescribing, to offer holistic care
Work within practice guidelines, policies and professional standards
Main duties of the job

Youll work as part of a supportive nursing team, providing holistic, personcentred care to a diverse patient population. Youll be guided by the Lead Practice Nurse and will uphold the Nursing and Midwifery Council code to ensure safe, effective practice at all times.

As a practice nurse you will be
expected to always maintain patient confidentiality, professionalism, empathy,
excellent communication skills, cheerful and polite manner. You will be
supported and supervised by the lead practice nurse within CHPL. As a practice
nurse you will adhere and uphold the professional standards set by the Nursing
and Midwifery Council code of conduct.
