Job summary
City Health Partnership Ltd (CHPL) is seeking a highly
motivated and enthusiastic Senior Practice Nurse with a V300 Independent
Prescribing qualification to join our team. The role involves the clinical
assessment and management of patients primarily within a general practice
setting, focusing on long-term condition management, including asthma,
diabetes, COPD, and hypertension.
Consultations will be conducted face-to-face, by telephone,
or via digital platforms, managing undifferentiated illness and acute
presentations related to long-term conditions. The successful candidate will
have a strong background in general practice nursing, with a thorough
understanding of QOF (Quality and Outcomes Framework) requirements,
immunisation programmes, and current health campaigns.
The Senior Practice Nurse will ensure the highest standards
of clinical care across CHPL, working within their professional boundaries and
supporting the implementation of organisational policies and procedures. They
will work closely with the Clinical Team Leader to support and improve service
progression, contribute to service improvement initiatives, and mentor junior
nursing staff. This role demands advanced clinical skills, including
independent prescribing, to provide comprehensive, holistic patient care.
The service opening hours are Monday-Friday 8am-6.30pm and
it is expected contracted hours will be worked during these times.
Main duties of the job
The post holder is accountable for the delivery of safe, high-quality, and effective clinical care within primary care
medical services. They will provide leadership and support to all nursing staff, collaborating closely with other
leaders and the multidisciplinary team to achieve service priorities. Clinically, the role is focused on implementing
evidence-based practice to meet the needs of a diverse patient population, ensuring optimal health outcomes.
