Job summary:
The post holder will act
as an Operations Manager for SASSE 2 Primary Care Network (PCN) in North West
Surrey.
Whilst employed by NICS
(the GP Federation) the role will report to, and be accountable to the PCN
Clinical Director and the PCN Board.
The role will support
the Clinical Director and PCN with managing the implementation of identified
projects, supporting achievement of Impact & Investment Fund (IIF) targets
and monitoring PCN development of community services in partnership with other
services such as social care, mental health and the voluntary sector.
The role will involve
day to day liaison with the PCN Practices to ensure engagement and
understanding of the PCN specifications and implementation.
Main duties of the job
Developing strong relationships and engagement
with a range of stakeholders across the PCNs, Community Providers, Member GP
Practices and Locality.
Developing plans for projects identified by the
PCN and support the Clinical Director with the implementation of any project
agreed by the PCN Board.
Developing
and implementing a rigorous system to track progress with PCN Practices against
the IIF targets set by NHSE; (including claiming on CQRS
and putting in appeal if needed.
Regularly
report targets, presenting to the Clinical Director and PCN Board on the
progress with ongoing projects, implementation of agreed patient services and
IIF targets.
Responsible for disseminating new information
across the PCN member practices and co-ordinating shared learning;
Understanding the PCN finances, in liaison with
NICS Financial Controller and Accountancy team, and reporting back on the PCN
financial position to the PCN;
Supporting other PCN Operations Managers to ensure
good practice and ways of working are shared and lessons are learnt.
Arranging PCN meetings including
writing and sending out the minutes., agendas and action plans
Attend all PCN
Board meetings and Ops Team meetings
Oversight, auditing and
co-ordinating CAIP (and any other mandatory contractual requirements)
Completing workforce reporting
templates for PCN (due twice a year)
Updating the NWRS portal
End of year PCN reporting to
ICB
Co-ordinating
staff surveys

SASSE 2 PCN (also known as SASSE Network 2) is a Primary Care Network in North West Surrey, England. It is a collaboration of five GP practices working together within the Surrey Heartlands Health and Care Partnership to provide integrated health and social care services to their local population.
