Job summary
SASSE 2 PCN are an established network of five GP
practices with good inter-practice relationships in the Staines, Egham and
Ashford area in the North West region of Surrey Heartlands Health and Care
Partnership.
We are looking for a further
Clinical Pharmacist to join our growing pharmacy team. The role involves
working not only in the practice but also with patients in the community to
deliver clinical excellence; including the National DES work and General
practice work.
Primary Care is a rapidly changing
environment and we are looking for someone who is adaptable and has the ability
to work as part of a GP led Multidisciplinary team. We understand the value of
Clinical Pharmacy in chronic disease management, timely structured medication
reviews and proactive management of patients with complex polypharmacy and see
the role allowing development of this.
The ideal candidate must have a
flexible can do attitude with a passion for clinical excellence and an
ability to work well within a team. This is an interesting and challenging
role, pivotal to the patient care and is likely to develop as the PCN matures.

Main duties of the job
The post holder will:
Support the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES
Support the delivery of medicines safety agenda
Support the delivery of enhanced care in care homes
To have made an effective contribution to the PCN Supporting the delivery of the pharmacy and medicines optimisation elements of the network specifications within the

OVERVIEW and SPECIFIC RESPONSIBILITIES
Clinical Pharmacy Service (CPS) The CPS will be delivered 5 days a week in the PCN. The clinical pharmacy service will be primarily patient facing and clinical in nature focused on improving medicines optimisation and safety working as part of a multidisciplinary team. Main elements include
Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications reviews.
- Undertaking structured medication review and optimisation to tackle over-medication of patients, in particular elements will include: Inappropriate use of antibiotics,Withdrawing medicines no longer needed,Structured medication reviews to support patients to take their medicines to get the best from them, reducing waste and promoting self-care.
Dedicated focus on priority groups:
Care Home residents
Frail elderly
Patients with complex needs taking large numbers of different medicines.
Asthma / COPD

- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
Experience

- Previous experience working within a patient facing role
