Job summary

This role would suit an experienced nurse prescriber, that using their advanced autonomous clinical skills, and broad and in-depth theoretical knowledge base, would be ableto deliver a high standard of patient care as Frailty Matron in general practice.

This role will manage a clinical caseload, dealing with presenting patients needs in a primary care setting or the patients home.

Main duties of the job

  • Manage caseload of patients with a range of medical need
  • Provide a service via telephone, face to face and in the patients own home
  • Use a range of different assessment methods such as physical examination, ordering and interpreting diagnostic tests or advanced health needs assessment, to establish a diagnosis and management plan
  • Make professionally autonomous decisions for which he/she is accountable
  • Make decisions related to patients medical management of both chronic and acute conditions
  • Manage issues related to frailty and care needs
  • Plan future care with a holistic approach involving both patients and families
  • Provide care to protect patients dignity and comfort throughout active management and end of life stages of care
  • Draw on a diverse range of knowledge in their decision -making to determine evidence-based therapeutic interventions (which will usually include prescribing medication and actively monitoring the effectiveness of therapeutic interventions)
  • Plan and manage complete episodes of care, working in partnership with others, and delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients
  • Prescribe safe, effective and appropriate medication as defined by current legislative framework
  • Provide safe, evidence-based, cost-effective, individualised patient care

Watergate Primary Care Network is made up of 3 friendly practices, Narrowcliff, Newquay Health Centre and Petroc Group Practice, which is a total of 6 GP surgeries, representing approximately 48,000 patients.
