Job summary

Job Summary

To assist medical personnel in the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.

A uniform and name badge must be worn at all times, which will be supplied by the Practice.

Youwill act as a focal point of communication between patients, doctors, nurses and other members of the primary health care team

Main duties of the job

Nursing

BP monitoring, including 24HR BP monitoring

Urinalysis

ECG recording and monitoring

Immunisations. Flu, B12 and pneumonia as per competencies

NHS health checks

Chronic disease work ups

Wound management, including dressings, clip and suture removal

Phlebotomy

Provide lifestyle advice to patients making any necessary referrals within the practice or to outside agencies

Weight monitoring

Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed

Ensure clinical waste collection runs smoothly, reporting any problems to the nurse manager

Ensure sharps bins are ready for collection

Restocking/maintenance of equipment

Restocking of clinical areas and consulting rooms

Job responsibilities

Administration and professional responsibilities

Participate in the administrative and professional responsibilities of the practice team

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes

Ensure the clinical computer system is kept up to date with accurate details recorded

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice

Attend and participate in practice meetings as required

Assist in formulation of practice philosophy, strategy and policy

Restocking and maintenance of clinical areas and consulting rooms

To assist in seasonal and special projects as requested e.g. flu campaign

Ordering and display of health promotion materials

Management of appointment system

Ensure total familiarity with all appointment systems including regular and incidental variations

Special Requirements for the post

An understanding, acceptance and adherence to the need for strict confidentiality

An ability to use own judgement, resourcefulness, common sense and local knowledge, to respond to patients enquiries and requests while adhering to practice limitations

An understanding and acceptance of ones capabilities and awareness of own limitations

Effective communication with clinical and non clinical colleagues to ensure the smooth running of the practice and efficient, safe, high quality patient service

Liaison with members of the wider PHCT and community services, CCG and secondary care in order to assure appropriate care is provided for patients.

Communicate effectively with patients and carers recognising needs for alternative methods of communication, with awareness and sensitivity to confidentiality, need for appropriate information and consent.

To follow Practice policy regarding the use of e-mail and computer tasks as the main internal method of non-verbal communication.

To listen to feedback from patients and colleagues whether this be a complaint, comment or compliment and if it cannot be dealt with immediately, ensure it is reported immediately to a member of the Practice Management Team

Team work

Understand own role and scope in the Practice and identify how this may develop over time

Participate in regular team and practice meetings, educational and development activities.

Maintaining quality and accuracy of all data input and clinical coding.

To work within the guidelines set byCQC to ensure that the Practice is safe,efficient andprovides the best possible care for patients.

To offer suggestions to the Managing Partner on how we can improve, progress and develop all processes relating to CQC.
