Job description

Job responsibilities

  • Conduct comprehensive clinical medication reviews for patients with complex needs (e.g. diabetes, hypertension, COPD, asthma, heart failure).
  • Deliver face-to-face and telephone consultations.
  • Respond to medication queries from patients, carers, and healthcare professionals.
  • Participate in clinical audits and quality improvement projects.
  • Manage clinical correspondence and monitor outcomes, liaising with secondary care and community services.
  • Maintain accurate records of clinical interventions.
  • Stay up to date with developments in primary care medicine.
  • Oversee repeat prescription systems, acute requests, and safer prescribing initiatives.

Qualifications

Essential

  • Masters degree in Pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Member of the Royal Pharmaceutical Society (RPS).
  • Independent Prescribing qualification.
  • Enrolled in or completed an approved 18-month Clinical Pharmacist training pathway (e.g., CPPE).

Experience

Essential

  • Experience managing chronic diseases and conducting clinical medication reviews, especially for complex patients.
  • Excellent communication, interpersonal, and organisational skills.
  • Ability to work independently and as part of a team.

Desirable

  • Postgraduate qualifications (e.g. Postgraduate Diploma in clinical, community, or therapeutic pharmacy).
  • Familiarity with SystmOne clinical system.
  • Additional qualifications in chronic disease management.
  • Knowledge of QOF, IIF, and PCN-directed services.
  • Previous experience in general practice.
