Job summary

We are seeking an experienced and autonomous Advanced Clinical Practitioner to join our multidisciplinary team.You will play a key role in delivering high-quality, patient-centred care and driving service development.

Main duties of the job

The successful candidate will deliver high-quality, patient-centered care through advanced clinical practice, including comprehensive assessment, diagnosis, treatment, and follow-up care. Working both independently and collaboratively, the ACP will play a key role in enhancing service delivery and improving patient outcomes.

  • Conduct comprehensive patient assessments, consultations, and physical examinations independently.
  • Diagnose and manage a wide range of acute and chronic health conditions.
  • Prescribe medications, order investigations, and initiate appropriate treatments in line with clinical guidelines.
  • Maintain accurate, timely, and confidential clinical documentation.
  • Collaborate closely with the multidisciplinary team to ensure coordinated, holistic care.
  • Contribute to service development, clinical governance, and evidence-based practice initiatives.
  • Support the training and mentorship of junior staff, students, and peers.
  • Participate in audits, quality improvement projects, and ongoing professional development.

  • Experience in General Practice and Systmone
