Heald Green Health Centre is looking for an experienced and dynamic Advanced Nurse Practitioner (ANP) to join our friendly, supportive, and forward-thinking primary care team.

Main duties of the job
The ANP/ACP will:
Provide high-quality autonomous clinical assessment, diagnosis, treatment, and management of undifferentiated, acute, urgent, and chronic presentations.
Undertake acute illness and disease management, including prescribing and medication reviews within scope of practice.
Offer same-day care and support urgent on-the-day demand as part of the duty/on-call team.
Provide first-contact care for minor illness, injury, and undifferentiated conditions.
Request, interpret, and action investigations as appropriate.
Support proactive management of long-term conditions, chronic disease reviews, and preventative healthcare.
Deliver face-to-face, telephone, and electronic consultations depending on practice needs.
Liaise with other health and social care professionals and make appropriate referrals.
Participate in public health and screening programmes including immunisations and vaccinations.
Contribute to clinical governance, service development, quality improvement, and care pathway development.
Provide leadership, mentoring, and specialist clinical advice to colleagues across the multidisciplinary team.

- Registered Nurse (NMC).
- MSc or Postgraduate Diploma in Advanced Clinical Practice (or equivalent experience).
- Independent Prescribing qualification.
- Evidence of ongoing CPD.
Desirable
- Minor Illness qualification.
- Advanced clinical assessment/decision-making training.
- Expertise or interest in specific long-term conditions (e.g., asthma, COPD, frailty, diabetes).

- Experience working in general practice and within multidisciplinary teams.
