Job summary

We are seeking a motivated and compassionate Nurse Associate to join our friendly and progressive team at Church Lane Surgery. This is a fantastic opportunity to be part of a multidisciplinary team delivering personalised, high-quality care within the heart of the local community.

Main duties of the job

The Nurse Associate will support the clinical team in delivering care across a wide range of services within the GP practice/PCN. Under the supervision of registered nurses and GPs, you will play a key role in health promotion, disease prevention, and delivering patient-centred care.

You will be a team player, willing to learn, and looking for a fresh and rewarding challenge. You will receive ongoing personal development and training opportunities in our high achieving and supportive environment.

About us

We are a rural practice, with over 12,300 patients, working from a modern purpose-built premises.

We are also a training practice with a wide multidisciplinary team including first contact physiotherapists, a PCN pharmacy team, Health & Wellbeing Coach, Social Prescriber, Mental Health Practitioner, and an in-house dispensary.

Job description

Job responsibilities

This is a 12-months fixed term position, 29-35 hours per week, with as soon as possible/immediate start date.

Responsibilities: This is not an exhaustive list

Support chronic disease management clinics e.g. asthma, diabetes, hypertension

Perform clinical observations, wound care, phlebotomy, ECGs and health checks

Administer vaccinations and immunisations in line with national guidance

Support cervical screening

Promote self-care and healthy lifestyle choices through patient education

Maintain accurate, timely documentation and contribute to care planning

Participate in quality improvement and population health initiatives across the PCN

Work collaboratively with the wider MDT to ensure high standards of care

Cervical Screening (if trained)

Qualifications

  • Registered with the NMC as a Nurse Associate
  • Completion of a Nursing Associate Foundation Degree
  • Strong communication, interpersonal and clinical skills
  • Ability to work both independently and within a team
  • Passion for delivering holistic, person-centred care

Desirable

  • Experience in a primary care or community setting
  • Training or experience in chronic disease management
  • Cervical screening (smear) trained and accredited
  • Familiarity with SystmOne
