Job description

Job responsibilities

To provide high quality care based on the needs of the practice population which includes treatment, screening, preventative care and patient education

To assess, plan, deliver and evaluate care holistically

To deliver care in accordance with agreed practice protocols and guidelines

Assist in developing new services and complementing and extending the range of services offered to patients

1. RESPONSIBILITIES

Key areas of responsibility will include:

Provide treatment and care in conjunction with the GPs or independently as appropriate and according to practice policy and guidelines

Take responsibility for own clinics and time management

Review and filing of own test results and those relating to chronic disease

Advise patients on general health and minor ailments with referral to the GP as necessary

Immunisations and vaccinations including childhood immunisations

Holiday vaccinations and travel advice

Venepuncture

First Aid for presenting emergencies

Diagnostic tests such as ECG, urinalysis, swabs, blood tests

Responsibility for the nursing management of patients with chronic disease/long term conditions including diabetes, CHD, CVA, asthma, COPD, CKD, mental health, learning disabilities, autism and hypertension.

To initiate, develop and implement protocols in relation to patients with chronic disease

To carry out cervical cytology

Carry put new patient health checks, well man and well woman checks

Actively promote health education and provide smoking cessation and weight management advice

Provide family planning and contraception advice

Act as chaperone where required

To promote and run health campaigns as appropriate

Ensure the drugs cabinet and doctors bags are monitored and kept up to date

To carry out risk assessments in accordance with COSHH regulations

Keep up to date with practice policies and procedures, especially in relation to recording systems, administration of drugs, child protection, confidentiality, health and safety

Adhere to infection control procedures and ensure practice policies are up to date

Cleaning of equipment and disposal of waste

To be competent in the use of the computer ensuring patients details are correctly input in accordance with practice guidelines

To actively contribute and oversee QOF targets directly relating to the practice nurse role

To provide vaccination to patients in their own home where required

Support PCN initiatives when required

3. AUDIT & RESEARCH

Collect routine data and participate and initiate clinical audit

To implement audit and research findings to improve clinical practice

To maintain disease registers

4. RELATIONSHIP

Responsible to the Partners for clinical issues and the Practice Manager for administrative issues

To work closely with other members of the nursing team, practising good teamwork at all times

Liaise with other team members e.g. admin and reception staff, District Nurses, Health Visitors, Community Matrons, School Nurses etc.

Liaise with secondary care and social services to ensure safeguarding, quality and continuity of care

5. ACCOUNTABILITY

Maintain own annual registration with the Nursing and Midwifery Council and work within the scope of the NMC guidelines

Be personally accountable for own practice and promote a professional approach to patients and staff

Acknowledge any limitation in knowledge and competence and practice within own limits at all times, highlighting any training needs to the Practice Manager

Maintain accurate records in line with NMC guidelines including data collection, completion of templates and read coding

Maintain own continuous professional development and ensure revalidation and PDP is kept up to date in line with the requirements of the practice and the NMC

Maintain own car insurance ensuring cover is in place for work related activities

6. PRACTICE DEVELOPMENT

Attend and contribute to meetings both internally and externally as required

Participate in Practice Development and planning

Attend courses and training as identified at appraisal and agreed with the Practice Manager

Co-operate/participate with research within the practice

7. PREMISES

Be aware and notify the Practice Manager of any Health and Safety and hazardous conditions

To be familiar with procedures if the fire alarm is raised and assist patients out of the practice without putting own life at risk

Ensure safety procedures are carried out in order to maintain a safe environment for employees, patients and visitors.
