Job summary
Situated in Walkden, we are a successful
practice with a branch surgery next door and a patient list size of almost
10,000. Our practice falls within Greater Manchester ICB and we are part of the
Walkden and Little Hulton PCN.
We are a patient centric practice
with a family orientated approach and are keen advocates of Greener Practice
credentials.
Our multidisciplinary team
includes 4 GP Partners, 8 salaried GPs, 1 practice nurse, 2 healthcare
assistants. We have a pharmacy team as
well as an experienced administrative and reception team. Our ARRs roles
include a physiotherapist, care coordinators, specialist gynaecology and
diabetes clinicians.
We are currently undertaking a
huge transformative project whereby all patients will be allocated their own
GP. We are totally committed to
continuity of care for our patient population
As an accredited training
practice, we provide education for GP trainees and medical students. Our
Partners are experienced trainers committed to mentoring the next generation of
clinicians.

- Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being
- Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- (not essential criteria for this position but desirable) To prescribe, after successfully completing an approved prescribing programme and once this is recorded in the NMC professional register. Prescribing must only be carried out within the employees scope of practice and level of experience and competence. The employee must only prescribe from the formulary linked to his/her recorded qualification and must comply with the statutory requirements applicable to his/her prescribing practice. Prescribing practice should always be evidenced based and respond to relevant local and national guidance. The employee is accountable for his/her decision to prescribe and must do so only where they have the relevant knowledge for the patients/clients health and medical history.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols

- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
