Care Navigator
Responsible to Operations ManagerOur Care Navigators provide the first point of contact for our patients and act as a focal point of communication between patients, doctors and other medical staff.
Care Navigators are key members of The Quarter Jack Surgery team. They are trained to work with all the resources available to them which may include signposting patients to another service, offering self help advice or booking with an appropriate clinician in the practices multi-disciplinary team. This work is often referred to as care navigation.
The following are the core responsibilities of a Care Navigator. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.

Ability to maintain confidentiality
Good telephone manner and inter-personal skills
Good standard of general education
Ability to work with a methodical and careful approach
Flexibility to adjust work patterns to meet additional workload as it arises
Ability to remain calm and well-organised under pressure
IT-literate, able to use email effectively, with basic knowledge of web-based and windows-based systems.
Able to multi-task and prioritise
