Main duties of the job

The Advanced Nurse practitioner will be responsible for a number of clinical areas such as Minor illness and injury assessments, health promotion, chronic disease management, as well as supporting the management team in the reviewing of clinical policy and procedure.

Job description

Job responsibilities

On a typical day, you will have your own patient appointment list and be responsible for managing your own workload, including appropriately triaging patients. As an Advanced Nurse Practitioner in Primary Care, you will support the duty team as part of a collaborative environment with GP and Advanced Practitioner supervision. You will see patients face-to-face or contact them by telephone during minor illness and injury clinics, providing treatment and making necessary referrals to other members of the primary or secondary healthcare teams. You will also see patients for chronic disease management, providing ongoing care, monitoring, and support for long-term conditions. You will undertake community visits as an autonomous practitioner, supported by the duty team. This includes supporting the care of complex patients in their own homes, including those in nursing or residential accommodation, as part of a multidisciplinary team.

The following are the core responsibilities of the advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels:

  • Attending multidisciplinary, clinical, and staff meetings.
  • Undertaking health reviews e.g. for patients with learning disabilities or dementia etc, resulting in the production of health action plans.
  • Assisting in the development of advanced care plans for frail or end-of-life patients, in collaboration with care coordinators and other teams.
  • Making direct referrals to primary, secondary, and social care services in line with locally agreed pathways, guidelines, and protocols.
  • Helping or leading projects such as vaccine rollouts in care homes and housebound patients.
  • Taking on lead roles within the surgery e.g. Carers lead.
  • Keeping up-to-date with mandatory training.
  • Working with the surgery team to better patient care e.g., achieving Quality and outcome frameworks are achieved.
  • Develop, implement and embed health promotion and well-being programmes
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence and local guidelines.
  • Provide wound care to patients
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
  • A willingness to train and learn new skills, supported by the team and supervisor.

In addition to the above responsibilities, the Advanced Nurse Practitioner may be requested to:

  • Act as the audit lead, effectively utilising the audit cycle
  • Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
  • Monitor and ensure the safe storage, rotation and disposal of medicaments
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice
  • Develop an area of specialist interest, taking the lead within the practice
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis
  • Drives the development of nursing services within the practice, liaising external agencies and professional organisations as required
  • Develops practice administrative and clinical protocols in line with the needs of the patient and current legislation

  • 1 Registered Nurse with valid NMC registration
  • 2 Masters level qualification in Advanced Clinical Practice or equivalent
  • Independent Non-Medical Prescriber
  • 3 Proven experience working autonomously in a primary care or urgent care setting
  • 4 Skilled in clinical assessment, diagnostics, and treatment planning
  • 5 Excellent communication and interpersonal skills
  • 6 Strong commitment to patient-centred care and evidence-based practice
  • 7 Ability to work effectively within a multidisciplinary team
  • 8 Strong organisational and time-management skills
