Job summary
North Wood Group Practice, is looking to recruit a Practice Nurse to join our well-established
and friendly team.

our GPs are strong Pharmacy, Nursing and Administrative Teams. We are a medical training
Practice, with a number of trainees well supported and integral to the team at any one time.
Our team is cohesive and approachable. There are regular meetings, training and support
for all.
Main duties of the job
The post holder will provide the full range of primary care to our patients, including:
immunisations, chronic disease management, cervical cytology, wound management, and all
the normal duties of a Practice Nurse.
You will work collaboratively with the whole general practice team to meet the needs of our
patients.
About us
North Wood Group Practice, is a two branch Practice. One branch is located in Tulse Hill
(SW2 3DX) and the second branch is at Crown Point (SE19 3NY). We rotate staff through
both branches on a fixed timetable basis, we operate as one team across both sites.
One of our branches has new premises and staff parking is available at both sites. We are
on all local bus and train routes.
We have a growing list size of 15,500 patients and are a 4 partner practice, including a
Pharmacy Partner. We are rated Good by the CQC and are striving for Outstanding.
We use EMIS, AccuRx and Docman 10 and have a consistently high QOF achievement. We
are active members of the local PCN and have access to all of the PCN associated
professionals.
We have a strong multidisciplinary team, which includes Pharmacists, Pharmacy
Technicians, Nurses, HCAs, and Advanced Clinical Practitioners.

- Requesting pathology tests, for example urine culture, swabs
- Interest in Diabetes and Coronary Heart Disease and Contraception
- Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
- Assist in and perform routine tasks related to patient care as directed by other clinicians
- Be integral to helping the Practice deliver specialist primary care nurse led services including
chronic disease management. Initiate the role of diagnosis for patients suspected to have a
chronic disease e.g. diabetes, COPD, asthma and CHD, referring to other clinicians as
appropriate.
- Manage programs of care for patients with chronic diseases by planning, providing and evaluating
care under agreed guidelines and patient group directions. Maintain disease registers in liaison
with administrative staff. Undertake annual review of patients.
- Administer child and adult immunisations and vaccinations in accordance with national and local

programs. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation
and disposals of vaccines and drugs, and that adequate stock levels are maintained.
- Undertake autonomous comprehensive assessment of patient need and develop care plans in
accordance with local and national protocols.
- Deliver care to specialist groups in accordance with practice protocols and QOF guidance, local
and national guidance.
Provide health screening to the Practice population.
Safely use equipment used in examination and treatments.
Computer literate with practice patient record system and electronic communication systems.
- Safely manage hazardous biological substances.
Communicate effectively with patients, relatives and carers regarding treatment and progress of
patients, maintaining confidentiality of all patient information.
- Manage issues arising from sensitive situations e.g. unwanted pregnancy, sexually transmitted
infection, child protection issues, and mental health issues.
- Act as patient advocate.
- Awareness and provision of evidence based care.
- Physically handle patients to assess and treat in emergency situations.

- Maintaining and cleaning equipment used by the nurses and GPs
- Maintaining GP and Nurses rooms, stocking and rotating items as required
- Maintain general tidiness and cleanliness of nurses and treatment rooms

- Ordering of vaccinations to maintain stock levels
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate records
