The new care coordinator role has been developed to support patients with complex care needs, ensuring they recieve effective, efficient and joint up health and social care.
The role will be integral in engaging, empowering and promoting self management as part of the NHS Long Term Plan to make personalised care 'business as usual' for our patient population.

Main duties of the job
To
work with the GPs and other primary care professionals to identify and
manage a caseload of patients; where required/appropriate, to refer patients
to other health professionals within the PCN.
To
help patients to manage their needs through answering queries, making and
managing appointments, and ensuring that patients have good quality written or
verbal information to help them make choices about their care.
To assist patients to
access self-management education courses, peer support or interventions that
support them in their health and wellbeing and increase their activation level.
To
maximise digital tools for patients and their carers across health and care services.
To raise awareness and share decisions with the GPs
and care professionals in MDT clinical meetings, identifying patients who
may benefit from support.
To raise awareness amongst clinicians of how to identify patients who
may benefit from shared decision making.

of the role and as the post holder develops there will inevitably be changes in
the emphasis of duties. It is expected
that the post holder will recognise this and adopt a flexible approach to work
and be willing to participate in training.
About us
North Wood Group Practice was formed in 2017 following the merger of
Tulse Hill Surgery and Crown Dale Medical Centre. The practice area
extends from Tulse Hill to Upper Norwood in South-East London.
Our North Wood Group @Crowndale site has recently undergone a major refurbishment to expand our services and provide more opportunities excel in health and social care.
There
are currently 15,500 patients registered at the Practice making it one
of the largest in the London Borough of Lambeth. The local population is
remarkably diverse in terms of culture, ethnicity and socioeconomic
status. We have a growing team of clinical and admin team members and
are always looking to be proactive to adapt and meet the needs of our patient
population.

Care Network (which also includes Herne Hill Group Practice, Brixton Hill Group
Practice, Knights Hill Surgery, Deerbrook Surgery and Lambeth Healthcare). They
will work closely with other primary and community care staff and healthcare
organisations to provide integrated services to our patient community.
