Fountain Medical Centre are looking for a part time/full time Advanced Nurse Practitioner.
We are looking for a dynamic and forward thinking ANP interested in a position to join our friendly well established training practice. We hope to recruit someone who will contribute to
providing high quality patient centered care to our patient population. Support
for CPD and special interests, holiday and study leave entitlement.
The ideal candidate should have excellent people, planning and organisation skills. You will need to show flexibility and able to work under pressure as well as being able to manage your own workload.
Successful candidates will receive an attractive package of competitive salary, commensurate with experience and qualifications, NHS Pension, allocated study leave in addition to 27 days annual leave (pro rata) plus statutory bank holidays.
Main duties of the job
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the PMS contract. Furthermore, the post-holder will adhere to the NMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team. The practice has weekly MDT meetings to encourage team working.

We are a dynamic, forward thinking training practice wishing to
deliver locally innovative and integrated services to our practice population
of approx 17500 patients. We are a SystmOne practice based in Morley, Leeds
and our last CQC rating was good.
Our clinical team of GP's, ANP, PA's, practice employed Pharmacist and large nursing team work alongside an evolving team of network employed pharmacists, pharmacy technicians, first contact Physiotherapists, care co-ordinators/social prescribers which reduce our workload.

- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
