Job summary
The Frailty Advanced Nurse Practitioner role is a pioneering
role, to help us develop a proactive approach to help us meet the needs of our
patients experiencing frailty in the community. This will be a key role in
helping lead on the delivering care provision for our frail and elderly
population including collaborating with the whole clinical team here at
Coleridge Medical Centre.
The main focus will be on acute and ongoing care for our
frailty population, including prescribing and deprescribing through urgent and
planned visits with caseload management. A passion for elderly care is vital
with an ability to deliver holistic advanced comprehensive assessments, care
planning and proactive preventative care.
Main duties of the job
The main duties of this role include but are not limited to:
Lead
on the delivery of care provision to our frail and elderly populations
including proactive Person-Centred Care Planning and collaborative working
Early
identification and recognition of deterioration to include proactive care and
escalation planning
Full
Comprehensive Geriatric assessment for residents that have moderate to severe
Frailty to support the coordination of care with community and urgent care
services
