Mayflower Medical Group is looking to recruit a 28 to 32 hour per week Practice Nurse to build on our growing Multi-Disciplinary Team.
We are looking to recruit a Practice Nurse who has experience within Primary care, and also has specialist chronic disease management experience and training, whether that be respiratory or diabetes.
The surgery is transitioning towards a traditional General Practice Model and we are looking to add to our enthusiastic nursing team to provide essential nursing care to our patients. We want to ensure a continuity of care for all patients with our nurses based on the ground at one of our 5 sites.
Main duties of the job
The Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, minor ailment treatment, immunisations, and health education. The Practice Nurse will work under the guidance and supervision of our team of advanced nurse practitioners, contributing to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.
The Practice Nurse is expected to perform clinical procedures and manage patient follow-ups, ensuring the delivery of care is in accordance with the latest clinical guidelines and within the scope of practice. This position requires excellent nursing skills, a patient-centred approach to care, and the ability to work effectively within a multi-disciplinary team to improve health outcomes.
