Job summary

We are seeking an experienced Advanced Clinicial Practitioner to work at Briercliffe Surgery.

Briercliffe Surgery has a registered population of over 8,000 patients with an active PPG group comprising of over 20 members.

We are a multidisciplinary practice that includes GPs, Paramedics, ANPs, Clinical Pharmacists, Practice Nurses, Health Care Assistants and Physiotherapists.

Main duties include AM and PM clinic comprising of 28 pre-booked appointments in total plus fair share of general clinical administration work and the occasional urgent appointment or visit.

You will be independent and work autonomously, proactive, motivated and approchable by the wider team.

To work as an independent clinical practitioner utilising advanced practice and prescribing without the need for supervision.

Main duties of the job

  • Provide care to patients, including those with undifferentiated and undiagnosed health problems
  • Provide continuing care to patients across the whole practice population
  • Provide a range of appointment types including econsultations, video, telephone and face-to-face
  • Experience in managing patients with complex needs and minor illnesses
  • Provide excellent clinical examination skills and communication skills
  • Applicants must have completed, in full, an advanced clinical practice qualification to post graduate or masters level including independent prescribing and have sufficient experience to work autonomously and independently within a wider team.
  • Act as an independent prescriber (preferred, though will support training and qualification if needed)
  • Able to demonstrate safe, clinical decision-making and expert care for patients within the general practice
  • Deliver a high standard of patient care using knowledge base with the ability to demonstrate safe, clinical decision-making and expert care for patients
  • Work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery of policy and procedures

About us

You will be based at Briercliffe GP Surgery. Our organisation has three Partners: Dr Jawaid Khan, Dr Mark Fuller and Mr Methven Forbes. However, the day to day running of the Practice is carried out by our inclusive board of senior team members that includes our lead clinicians.

The organisation has a board structure with senior members of the clinical and non-clinical team directly involved in the decision-making processes of the organisation through our Senior Management Team and Clinical Reference Group.

Job description

Job responsibilities

· Work as an autonomous clinical practitioner accountable for own professional judgement and actions

· Consult, assess, diagnose, plan, implement and evaluate treatment/interventions and agree goals for patients presenting with an undifferentiated diagnosis through a co-production approach

· Provide support with the e-consultation service to patients submitting e-consultations (in addition to general appointment delivery types)

· Proactively identify patients at risk of developing a long-term condition (as appropriate) and facilitate the creation of their own goal setting through the development of a self-management plan

· Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a patient management plan

· Undertake face to face assessments and develop treatment plans for patients not requiring a GP consultation

· Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice

· Request and interpret tests appropriate to patients needs

· Where required, undertake home visits as assessed by GP and acute visits for certain medical emergencies and arrange hospital admission for appropriate cases

· Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

· Undertake telephone triage work and telephone advice calls

· Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care

· Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation

· Assess, identify and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services

· Implement and participate in vaccination and immunisation programmes for both adults and children

· Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

· Communicate with and support patients who are receiving difficult or complex information or results in an professionally appropriate and supportive manner

· Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
