Job summary

An exciting opportunity has arisen at Millcroft Medical Centre for a Triage Nurse to join our team.

We are looking for an enthusiastic, motivated and forward thinking individual who can work as an autonomous practitioner but also work well with a large multidisciplinary team.

Main duties of the job

The Triage Nurse role is integral to ensuring that all patient requests into the practice are clinically assessed and prioritised accordingly.

The post holder will identify the clinical need and urgency to provide a timeframe for intervention, allocating them to the most appropriate clinician or service to meet the needs and will provide advice and signposting as required.

We are a growing practice with a population of over 30,000 patients working within a purpose built practice in Crewe, Cheshire. We are an EMIS web, DOCMAN, Accurx, paper light practice.

  • -Triage / clinical assessment Training
  • -Registered on their appropriate professional register
  • -Evidence of continuing post-registration development
  • -Telephone consultations skills training

Desirable

  • -Qualified independent prescriber - V300

Experience

Essential

  • -Experience of multi-disciplinary working
  • -Experience of telephone triage and face to face clinical assessment
  • -Experience of working in a primary care environment

Desirable

  • -Clinical supervision / mentorship
