Job summary

An exciting opportunity has arisen at Millcroft Medical Centre for a Advanced Nurse Practitioner to join our team.

We are looking for an enthusiastic, motivated and forward thinking individual who can work as an autonomous practitioner but also work well with a large multidisciplinary team.

The advanced nurse practitioner will be responsible for clinical areas such as consulting patients with acute medical problems, health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

This is an excellent opportunity for an enthusiastic and motivated ANP with a strong commitment to excellent patient care and an ability to work well as part of a well organised team.

Deliver high standard of patient care to the registered patient population using advanced clinical skills.

Manage a clinical caseload dealing with patients presenting to the practice.

Manage chronic disease clinics

Work with all multi-agencies for the benefit of the patient

To prescribe safe, effective and appropriate prescribing of medication.

Support the practice to develop the services provided to the patients.

We are a growing practice with a population of over 30,000 patients working within a purpose built practice in Crewe, Cheshire.

We are an Emis-Web, Docman , Accurx, paper light practice. Our current team has a full complement of allied health professionals including, ten GP Partners, Salaried GPs, Nurse Practitioners, Paramedic, Mental Health Workers, Practice Nurses, Physiotherapist, SPLW, Pharmacist’s, Business Manager, Practice Manager and a complement of highly skilled support staff.

To work as an autonomous practitioner, working as part of the practice multidisciplinary team, delivering care within their scope to the practice population. The advanced nurse practitioner will be responsible for clinical areas such as consulting patients with acute medical problems, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

-The delivery of highly effective medical care to the entitled population

-Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

-Implement and evaluate individual specialised treatment plans for chronic disease patients

-Prescribe and review medication for therapeutic effectiveness appropriate to patient needs and in accordance with evidence based practice

-Encorage with patients compliance and adherence to prescribed medication, providing advice and information on medication regimes.

-Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patient’s health

-Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence.

-Request pathology services as necessary

-Process and interpret pathology and other test results as required
