medical record and relevant information is coded.

administrative related tasks as delegated by the doctors and non- clinical
staff
Main duties of the job
The aim of this role
is to streamline the flow of clinical correspondence through the organisation,
by reducing the number of letters that GPs need to process. This should ensure
that GPs are able to give priority to the most appropriate correspondence. In addition this role may also need to be
able to provide cover for scanning person if needed.
The Docman Processor
will follow the Workflow Optimisation Flowchart. Where appropriate, according
to the flowchart, letters will be filed directly to patient notes by the Docman
Processor and where appropriate, more complex correspondence will be actioned
(by calling patients and arranging diagnostic investigation, liaising with
other healthcare clinicians, primarily pharmacist if change of medication is
needed) or letters should be directed to the appropriate clinician for their
attention. Where there is uncertainty about whether a document can be filed
directly, it will be forwarded to a clinician for safety.
In addition, the
post holder will identify any key clinical data from correspondence, such as
new diagnoses, peak flow measures, BP recordings, height, weight, smoking
status, alcohol intake status, blood test results, care plans etc.

It was situated in a house in the Millway until 1998 when it moved to the current 3 storey modern building in Hartley Avenue which it shares with community staff of Barnet Primary Care Trust who occupy most of the 2nd floor.

- To function as an integral part of the Practice team.
- To attend relevant clinical meetings as required.
- To have knowledge of and act at all times within the boundaries of Health and Safety legislation and Data Protection acts.
- To report immediately to a member of the clinical staff or the Practice Manager any acts of omission or error to safeguard the interest of the patient.
- To participate in the induction programme of staff new to the practice
Key
duties and responsibilities
Administration
To improve
efficiencies and reduce the number of people processing patient documents
To
add any additional information about the source of the document into the
patients medical record
Read
code accurately diagnosis, procedures and investigations, identifying and
carrying out actions e.g. contacting patients to make appointments
To
forward the documents to the appropriate team member for actioning which could
include GPs, Nurses, Pharmacist, Admin and Secretarial team etc
To
store documents in line with the GDPR and practice regulations
To
destroy documents in line with the GDPR and practice regulations
To
liaise with patients to inform them of actions taken e.g. appointments booked
or referral made etc
Follow
practice protocols to ensure consistent and high quality care is provided to
our patients
Carry
out administrative tasks relating to patient records, including filing and
document management
Identify
the complex or higher risk areas that require input or knowledge from the
clinical team, including Safeguarding issues, working to the agreed Practice
protocols.
Attend
any workflow optimisation training as deemed necessary by the practice
Identify
relevant clinical information contained in patient correspondence and code or
free text the clinical information onto the patient record
Deal
with queries relating to coding
Deputise
scanning duties - assigning documents to patients clinical record within 48
hours
Clinical Duties
To act as a
chaperone as required to GPs and Practice Nurses.
To
act in such a manner as to support the client/patient at all times and be
mindful of the need to reduce anxiety for the client/patient.
To
assist in general nursing duties, under the supervision of the Practice Nursing
Team, i.e. Urine testing, weight/height, blood pressure measurements,
spirometry, peak flow measurements, smoking cessation advice and any other
duties that may be assigned as necessary.
To
be responsible for the processing and management of laboratory samples
requested by the GP/ Practice Nurses with specific responsibility to ensure
that the samples are correctly labelled, sent to the laboratory promptly and
that data is entered onto the patients computer records.
