Job summary

Accurately examine and assess patients with an undifferentiated diagnosis, from a physiological, psychological and social perspective, and plan clinical care accordingly.

Examine undifferentiated and undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making to diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Job description

Job responsibilities

Accurately examine and assess patients with an undifferentiated diagnosis, from a physiological, psychological and social perspective, and plan clinical care accordingly.

Ensure full and accurate documentation in patient notes of clinical history, physical examination, diagnosis and treatment plan

Where the post holder is an independent prescriber, to prescribe safe, effective and appropriate medication as defined by current legislative framework.

To clearly communicate the health status of the patient using appropriate terminology, technology and format and maintain respect and dignity for the patient at all times.

To work within the practice guidelines, policies and protocols.

To maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldecott Principles in relation to all data handling.

  • Registered professional Emergency Care Practitioner / Paramedic / Primary Care Associate Resuscitation - minimum of Basic Life Support (BLS)
  • Significant post registration experience, to include: History taking Assessment Diagnostics Minor injury Recent clinical experience in related client group, i.e. community setting

Desirable

  • Working knowledge of SystmOne
