Job summary
Accurately
examine and assess patients with an undifferentiated diagnosis, from a
physiological, psychological and social perspective, and plan clinical care
accordingly.Examine
undifferentiated and undiagnosed problems, making use of skills in history
taking, physical examination, problem-solving and clinical decision-making to
diagnose and manage both acute and chronic conditions, integrating both drug-
and non-drug-based treatment methods into a management plan.
Prioritise
health problems and intervene appropriately to assist the patient in complex,
urgent or emergency situations, including initiation of effective emergency
care.Review
medication in accordance with PGD medication list as approved by the supervising
G.P.Provide
information and advice on prescribed or non-prescription medication, on
medication regimens, side-effects and interactions
Ensure
full and accurate documentation in patient notes of clinical history, physical
examination, diagnosis and treatment plan.To
clearly communicate the health status of the patient using appropriate terminology,
technology and format and maintain respect and dignity for the patient at all
times.
To
work within the practice guidelines, policies and protocols.
To
maintain accurate and contemporaneous records, utilising computer systems where
appropriate and consider the Caldecott Principles in relation to all data
handling.
Main duties of the job
The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.With appropriate clinical assistance, they will demonstrate safe, clinical decision-making and expert care for patients within the general practice.The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.The post holder will build effective working relationships with other health care professionals.The post holder will work within the clinical on-the-day team to deliver excellent primary health care services within the practice.
