The Blackmore Vale Partnership (BVP) is recruiting a Wellbeing & Transformational Lead who will focus on leading the wellbeing team and transforming services within the partnership. This will contribute to the overall progression of the Vale Primary Care Network (PCN) and is an opportunity to make a real difference to the lives of population within the partnership catchment area.

Main duties of the job

The role will form an integral part of the BVP leadership team working alongsidethe Clinical Director, Business Partner and colleagues to meet the requirements of the various PCN & Practice contracts. You will plan and deliver schemes relating to service specifications and initiatives using a quality assurance approach to review and improve existing services.You will also be expected to be able translate national and local policy into meaningful changes that will support the partnership to achieve improved outcomes for the local population.

You will lead and have responsibility for the management of the BVP Wellbeing service to offer a coordinated approach to support the personalised care agenda. This service works in collaboration with various stakeholders and partner agencies to offer a comprehensive provision from social prescribing to health and wellbeing coaches.You will be building relationships with all theGP Practice staff, NHS Dorsetanda wide range of otherhealth,social care, voluntary sector and otherprofessional colleagues.

The Blackmore Vale Partnership (BVP) is recruiting a Wellbeing & Transformational Lead who will focus on leading the wellbeing team and transforming services within the partnership. This will contribute to the overall progression of the Vale Primary Care Network (PCN) and is an opportunity to make a real difference to the lives of population within the partnership catchment area.

The role will form an integral part of the BVP leadership team working alongside the Clinical Director, Business Partner and colleagues to meet the requirements of the various PCN & Practice contracts. You will also be expected to be able translate national and local policy into meaningful changes that will support the partnership to achieve improved outcomes for the local population.

You will lead and have responsibility for the management of the BVP Wellbeing service to offer a coordinated approach to support the personalised care agenda. This service works in collaboration with various stakeholders and partner agencies to offer a comprehensive provision from social prescribing to health and wellbeing coaches.You will be building relationships with all the GP Practice staff, NHS Dorset and a wide range of other health, social care, voluntary sector and other professional colleagues.

MAIN RESPONSIBILITIES

Lead and manage the wellbeing team at BVP

Act as a strong advocate for the partnership when engaging in PCN work

Be responsible for one or more complex and high-profile projects, using planning, monitoring and controlling processes

Gain evidence to meet the NHS contractual requirements

Support the transformational work including the adoption of new technology and other initiatives to improve the care offer

Monitor the delivery of the wellbeing workstreams working with key stakeholders and clinical leads

Enhancing the integration of the various teams and services both within and external to the partnership

Monitor progress of projects against performance and project targets, identifying where corrective action needs to be taken.

Identify future project opportunities and create a workplan against these opportunities

Lead on the Population Health Management and associated workstreams to include: -

o Complete Care Community

o Aging well

o Risk stratification

o Personalisation agenda

o Digitalisation to support chronic disease management

Be fully aware of all available resources available to wellbeing service in the local community and beyond and continually add to the wellbeing service knowledge base

Network and liaise with a wide range of relevant services, community groups, schools, social care, housing, community teams

Maximising use of available data to identify groups most adversely affected by health inequality and contribute towards developing interventions that can effectively address these.

Collaborate with wider Social Prescribers and Wellbeing Team to understand the future developments of NHS ARRS roles (additional roles with responsibilities)

Participate in relevant training and professional development opportunities, cascading learning back to the wider partnership

Be familiar with, and operate within, all relevant BVP policies and procedures to always ensure safe working practice.

COLLABORATIVE WORKING RELATIONSHIPS

Recognises the roles of other colleagues within the organisation and their role to a persons care
