Job summary
Care coordinators play an important role within the practice
to proactively identify and work with people, including the frail/elderly and
those with long-term conditions, to provide coordination of care and support
across health and care services.
They work closely with GPs, practice and care home teams to
manage a caseload of patients, acting as a central point of contact to ensure
appropriate support is made available to them and ensuring their changing needs
are addressed.
They will be caring, dedicated, reliable and person-focussed
and enjoy working with a wide range of people. They will have good written and
verbal communication skills and strong organisational and time management
skills. They will be highly motivated and proactive with a flexible attitude,
keen to work and learn as part of a team and committed to providing patients with
high quality support.
They will be enthusiastic and hard-working with knowledge of
primary care and ideally 2 years experience working in a health or social care
related profession.
Main duties of the job

Effectively manage MDT meetings for care home residents and
produce minutes

Implement a process for ensuring patients have their cancer
screening
Implement a process for safety netting at the practice, so
patients are followed up effectively
Collate patient data with best practice and upload and
regularly update the patient record
Create, amend and use searches within the clinical system to
identify patients
Assist Enhanced Access with certain aspects of patient care
Work with the admin team to ensure patient registrations are
processed in a timely manner
Participate in practice meetings and training events
About us
Spring House Medical Centre currently has a list size of 9800 patients, we are slightly smaller than other local practices. We have a strong clinical team of 8 doctors and 3 nurses plus a very supportive admin team. We have a strong work ethic within the practice making sure patients receive the best care available to them. The team are very close and completment each other very well.
