Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Advanced Nurse Practitioner to join our friendly clinical team at Warwick Gates Family Health Centre.

We are looking for 25-30 hours, adding to our current team of experienced ANPs to support us in continuing to provide high-standard Primary Care to our local population in Warwick Gates and surrounding areas.

We have robust governance processes in place, including a rotation of weekly meetings and gatherings focused on practice quality & performance, safeguarding and vulnerable patients, staff/operations, significant events and clinical team collaboration.

Partners encourage a culture of transparency and openness to ensure learning from all feedback ad complaints received.

We have an active Patient Group who meet bi-monthly with the Practice Manager and other staff members. The Group are sighted on practice plans and objectives, opportunities to improve access, patient survey results and PCN priorities.

Main duties of the job

The successful candidates will:

- Carry out patient assessment, diagnosis as appropriate and adequately interpret test result

- Independently prescribe appropriate medication within scope of practice

- Evaluate or refer to other specialists where necessary

- Work alongside our existing ANP Team with regular pre-allocated clinical supervision sessions held with one of our GP Partners to ensure a supportive culture and continuous learning/development takes place.

- Provide high quality effective patient care

- Investigate and refer patients as appropriate in line with current NICE guidance

- Carry out both face to face and telephone consultations

- Ensure appropriate clinical coding to support clinical audit and Quality & Outcome achievements

- Ensure accurate and robust note keeping

- Ensure excellent communication skills with patients, carers and their families

- Ensure attendance and contributions at weekly meetings and educational sessions

- Build strong working relationships with Partners and the wider practice team ensuring collaboration with all staff

- Effectively manage own time, workload and resources

- Ensure compliance with on line mandatory training and attend annual prescribing updates

- Attend required training events e.g. PLT and feedback

- Proactively report and consider any Learning/Significant Events

- Share concerns and obtain feedback and clinical insight from GPs and peers where needed

About us

The successful candidate(s) will join a friendly and dynamic practice team that works together to serve a practice population of approximately 12,500 patients, aiming to deliver modern primary care with the traditional feel of a family GP Practice.

Due to local housing developments, our patient list size is increasing at a pace, with ongoing growth and sustainability at the centre of our strategic planning.

Our team consists of 3 GP Partners, 5 salaried GPs, 2 Advanced Nurse Practitioners, 2 Practice Nurses, 1 Nurse Associate, 1 Healthcare Assistant, 3 Medical Secretaries and a team of Care-Coordinators overseen and supported by our Management Team. We use EMIS clinical system and communicate daily using MS Teams.

We are part of South Leamington Primary Care Network and work proactively with neighbouring Practices to deliver services at scale, focused on local community needs.

Job description

Job responsibilities

The main responsibility of the practitioner will be to see patients requiring same-day acute and minor illness type appointments. This includes 15 patients per session, including face-to-face, telephone and home visit contacts. This may include extended hours sessions pro rata.

Undertake consultations with patients (including babies & children from 12 weeks) with a range of acute and chronic medical conditions as an autonomous practitioner. Using own clinical judgement to diagnose, treat, refer and/or discharge these patients.

Make direct referrals to acute, secondary care and social services within locally agreed pathways, guidance and protocols as required

Develop, implement and embed health promotion and well-being programmes & deliver such health promotion where appropriate

Implement and evaluate individual specialised treatment plans for chronic disease patients, referring patients to secondary care as required

Contribute to practice targets (QOF etc.), complying with local and regional guidance

Maintain chronic disease registers

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Request, process and interpret pathology and other test results as required

Ensure that patients receive high quality clinical care, delivered in a timely manner

Supply and administer medicines as indicated to address patient need, where necessary working to Patient Group Directive

Ensure complete and accurate documentation of each and every patient contact via EMIS web. Use read codes effectively

Work as an autonomous practitioner and as part of multi-disciplinary and multi-agency teams in order to ensure patients needs are met

Provide clinical advice to Reception staff where required

With other staff in the practice help deliver vaccination programmes for adults and children ie influenza

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Implement vaccination programmes for adults and children

Support the clinical team with all safeguarding matters, in accordance with local and national policies
