Job summary

You will work as part of an integrated nursing team, you will be working within a multi-disciplinary team. Your responsibility in a strong support role to the doctors will be to provide a variety of key functions. These include physical examinations, diagnosing illnesses, providing health promotion and education to patients and prescribing within parameters.

Main duties of the job

To carry out all general nursing duties when required, offering the highest possible standard of patient care and education.

To provide evidence based advice for patients, their families and caregivers specific to their diabetes management needs, empowering them to make informed choices about their diabetes care.

The assessment, planning, implementation and evaluation of programmes of care, for patients with asthma and COPD.

Ensure continuity of care by making sure that patients are referred to appropriate agencies for additional support or treatment at the most appropriate time smoking cessation services, pulmonary rehabilitation, community matrons, palliative care, social services and voluntary support groups.

Job responsibilities

DIABETES:

To provide evidence based advice for patients, their families and caregivers specific to their diabetes management needs, empowering them to make informed choices about their diabetes care.

To plan, assess, implement, and evaluate programmes of care for patients with diabetes that reflect peoples complex and changing health care needs. This will involve communicating with other health care colleagues to ensure that appropriate nursing care and education of people with diabetes is provided.

To ensure diabetes guidelines are followed within the Practice.

To be aware of National and local targets that impact on diabetes management.

To provide diabetes expertise in the inpatient setting.

To maintain accurate and comprehensive patient records.

To support the diabetes team in the development and improvement of the diabetes service utilising clinical, facilitation and leadership skills.

To participate in local and national networking groups such as Diabetes UK taking responsibility for own learning needs and sharing and disseminating best practice.

RESPIRATORY:

Responsible for the assessment, planning, implementation and evaluation of programmes of care, for patients with asthma and COPD.

Provide specialist advice and education to patients, carers and members of the multidisciplinary team regarding health promotion, treatment options and outcomes in respiratory disease.

Ensure continuity of care by making sure that patients are referred to appropriate agencies for additional support or treatment at the most appropriate time smoking cessation services, pulmonary rehabilitation, community matrons, palliative care, social services and voluntary support groups.

To order/arrange, perform and act on spirometry, chest x-ray and to take an in-depth medical history in order to diagnose the identified respiratory conditions and manage symptoms in line with appropriate guidelines and policies.

Use judgement to interpret patient test results, identify abnormalities, notify the GPs and take action regarding abnormalities.

Communicate with patients and their carers the complexities of administering nebulised medication in order that they can carry out these procedures independently and safely within their home setting.

Communicate contentious information where motivational, persuasive, reassurance and empathy skills are required e.g. compliance issues with treatment.

GENERAL NURSING: To carry out all general nursing duties when required, offering the highest possible standard of patient care and education. The list below gives a brief outline of some of the duties you are likely to be required to carry out as part of general nursing duties, however this list is not intended to be exhaustive:

Administer necessary injections

Administer travel vaccinations according to current guidelines and educate the patient with any relevant information such as possible side-effects and post-vaccination care

Provide patients with the necessary treatment for minor trauma and wounds

Provide patients with post-operative care

Provide leg ulcer care and treatment

Warfarin monitoring

Ear-Syringing using the appropriate method of treatment

Carry out venesection

The removal of splinters

Monitoring of blood pressure

The removal of sutures

Carry out HV swabs

Offer family planning advice when appropriate

Provide health checks in accordance with current protocol

Assisting GPs in carrying out minor operations and procedures as required

Give counselling to patients when deemed necessary

other GENERAL responsibilities:

To be aware of current research and participate in practice audits.

Responsible for a high standard of data input during patient consultation, using the correct recommended Read codes in order to maintain good quality records and fulfil the required targets i.e. QOF, CQRS

Manage time effectively, prioritise and organise workload.

Stock control and requisition of drugs, vaccines, dressings, lotions and other medical supplies.

Ensure that any forms, reports and paperwork is completed as necessary.

Ensure that vaccines are stored according to manufacturers guidelines.

Maintain, clean and check all medical equipment is in good working order.

Qualifications

Essential

  • In order to be successful, you will need to be registered with the NMC and ideally have previous experience of working as a practice nurse in Primary Care.
  • Recognised Diploma Qualifications for the management of Diabetes.

Desirable

  • The successful post holder will ideally be ARTP certified, however training can also be offered.

Experience

Essential

  • You will have previous experience and evidence of working with spirometry results.
  • Have extensive knowledge of diabetes having previously worked in secondary care or as a Practice Nurse with experience running clinics independently
