Job summary

Come and join our lovely team at Francis Grove Surgery. We are looking for a full or part time dedicated and skilled Advanced Clinical/Nurse Practitioner to join our supportive and friendly healthcare team. The role is initially to cover Maternity Leave but may well lead on to a permanent position in the team.

The successful candidate will play a pivotal role in delivering high-quality patient care, working collaboratively with other healthcare professionals to assess, diagnose, and manage patient conditions. This position requires a strong clinical background and the ability to work autonomously while also contributing to a multidisciplinary team.

Main duties of the job

Practice in compliance with respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice

Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change

Use expertise and decision-making skills to inform clinical reasoning

Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

Actively engage in peer review to inform own practice, formulating and implementing strategies to act on learning and make improvements

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Maintain accurate clinical records in conjunction with extant legislation

Contribute to practice targets (QOF, etc.)

Support the team in dealing with clinical emergencies

A little about the surgery and its philosophy and performance

Francis Grove Surgery is an established GP practice in the heart of Wimbledon. The practice is a 3 partner practice, with a list size of 14,700 patients. It is situated in modern premises on the ground, first and second floor in the Courtyard Development only minutes walk away from stations, shops, cafes and Wimbledon Shopping Centre.

Wimbledon is a vibrant and popular suburb of London with excellent links to other parts of London via underground and overground stations, buses and trams. Much of the area is made up of comfortable private housing with very little social deprivation. The practice population is well informed, comprising a large number of working professionals and their young families.

Francis Grove Surgery has an excellent reputation locally and is a popular practice. Feedback from patients is very good and they tend to stay with the practice.

The practice strives to maintain the core values of traditional General Practice and provide good continuity of care. We aim to provide a service which is responsive to the needs of the population with good quality care, respecting diversity and human rights. p. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

q. Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively

r. Develop, implement and embed an effective call/recall system

s. Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others

t. Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance

u. Chaperone patients where necessary

v. Support the team in dealing with clinical emergencies

w. Support the team with all safeguarding matters in accordance with local and national policies

x. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

y. Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the Advanced Clinical Practitioner may be requested to:

a. Support the IPC lead, ensuring compliance, and undertaking audit as necessary

b. Monitor and ensure the safe storage, rotation and disposal of medicines

c. Undertake overall management of a clinical team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

d. Maintain an effective clinical staff rota ensuring all clinics are staffed appropriately

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the organisation

g. Develop an area of specialist interest, taking the lead within the organisation

h. Continually review clinical practices, responding to national policies and initiatives where appropriate

i. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

j. Commensurate with their role, support the development of clinical services within the organisation, liaising with external agencies and professional organisations as required
