Job summary
Come and join our lovely team at Francis Grove Surgery in our ewly renovated premises. The post holder will be covering maternity leave and will be responsible for ensuring the delivery of safe and effective nursing care to the practice population. They will lead and manage the nursing resource, working closely with the practice management and GPs to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. The Nurse Lead holds joint responsibility for ensuring the practice achieves its full potential for QOF, PCN targets and other locally enhanced services involving nursing care. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team. The Nurse lead will be part of the Senior Management Team.
Main duties of the job
The Lead Nurse delivers Long Term Condition management as well as the full range of primary care nursing duties. They each have areas of practice that they lead in for example Diabetes, childhood imms, Asthma etc . They ensure plans are in place to ensure all relevant staff are trained appropriately and patients get the best possible care.
The Lead Nurse is a CQC lead, ensuring services provided by the nurse team are effective, safe, responsive, caring and well-led. They take responsibility for ensuring all relevant policies, procedures, risk assessment and best practice is following within the nurse team and the wider team for some areas.
The Lead nurse manages and appraises the nurse team, works closely with PCN staff such as pharmacists, Paramedics, social prescribers who deliver aspects of QOF/other targets.
The role ensures regular team and clinical meetings, they attend the practice governance meetings and support staff to attend relevant forums and training.
Ensure effective use of nursing appointments and that what is offered meets the needs of patients. Ensure we are utilising staff time in the best possible way.
Ensure all duties under the remit of the nursing team are undertaken.
Plan and Implement annual programmes and new PCN requirements or locally enhanced services that are relevant to the nurse team.

Francis Grove Surgery is an established, well-regarded PMS GP practice, doing our best to keep traditional family general practice at core of how we work, whilst embracing change and innovation. We are based in the heart of Wimbledon, only a few minutes from Wimbledon station and the high street with a list size of 14,700 patients.
The practice is part of the West Merton PCN, which is made up of us and one other practice. We have a productive working relationship and do our best to utilise resources and funding effectively to increase capacity and staff and patient satisfaction.
It is a great team to work within, very supportive, friendly and dedicated. The team are well respected within the wider practice team and are central to the practice delivering excellent service to the patients.
