Band 7/8a PCN Pharmacist for St Thomas Medical Group
We are excited to offer this post for a self-motivated and enthusiastic clinical pharmacist (band 7/8a depending on experience) to work alongside our PCN pharmacy team. You will report to the Additional Role Clinical lead GP, and the Clinical Director for the network.
As an experienced pharmacist you will be based in the St Thomas Medical Group, as an integral part of the PCN pharmacy team working across the whole of the PCN as needed. The team comprises two other pharmacists and two pharmacy technicians, both in-house, and remote.

The PCN has an internal culture of support for learning and academic progression, and you will be supported to develop your educational goals and advance your clinical pharmacist skills. We have a robust and inclusive supervisory structure with a GP dedicated to overseeing and supporting the academic and clinical progression of our PCN Additional Roles. You in turn will be involved in the oversight of trainee pharmacists and allied health professionals where needed.
You will be working as part of a wider practice and PCN team including GPs, nurse and paramedic prescribers and physios. You will provide support to practice clinical and non-clinical staff responding to prescribing and medicines queries, medicines reconciliation on transfer of care and long-term condition management. You will carry out patient phone and face-to-face consultations, to conduct medication reviews. You will also support medicines safety systems, and the medicines optimisation agenda, while delivering high quality care,quality improvement and helping to deliver QOF processes within the practice IT system.
You will demonstrate courageous and critical thinking, and be a caring, compassionate and committed clinical pharmacist. Acting within your professional boundaries you will provide support for the patients of the practice as part of an integrated team.
