Job summary

We are a friendly, busy, growing GMS practice in Melksham, working from a purpose-built Medical Centre.

You have a unique opportunity to join our high achieving, forward thinking Clinical Team in shaping the future of primary care.

We have a multi skilled, very experienced team and are looking for an Advanced Clinical Practitionerto work between 27 - 37.5 hours a week.

Main duties of the job

You must be able to work autonomously carrying out clinical, duty minor illness consultations providing assessment, diagnosis, treatment and referral of patients. Ideally, you will hold a non-medical prescribing qualification or prepared to work towards achieving this, be confident to assess patients of all ages, including paediatrics. You should be fully committed to providing excellent patient care we expect team members to have a can-do attitude, which puts the patient first.

About us

You will be fully supported by a broad clinical team comprising of GPs, Pharmacists, Paramedics, Practice Nurses, Advanced Nurse Practitioner, Physio, Mental Health Practitioner, Health Care Assistants and Phlebotomists. This is a permanent role. Full induction and training is given on joining. We offer a generous NHS pension scheme, staff uniform, five weeks annual leave and bank holidays pro rata.

Job description

Job responsibilities

The following are the core responsibilities of the Advanced Clinical Practitioner/Duty Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Develop, implement and embed health promotion and well-being programmes.

Triage and manage patients presenting with a range of acute medical conditions, providing subject matter expert advice to patients and referral pathways.

Face to face, telephone triage, telephone consultations, and home visiting which will enable patients to obtain health advise.

To accurately identify those patients at risk of abuse and a good working knowledge of support and referral pathways.

Have a good knowledge of safeguarding policies in relation to both children and adults.

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health.

Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and the NSF.

Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

Undertake the collection of pathological specimens.

Request pathology services as necessary.

Process and interpret pathology and other test results as required.

Provide acute problems / emergency clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.

Maintain accurate clinical records in conjunction with current legislation.

Ensure read codes are used effectively.

Chaperone patients where necessary.

Prioritise health issues and intervene appropriately.

Support the team in dealing with clinical emergencies.

Recognise, assess and refer patients presenting with mental health needs.

Extended and supplementary prescriber, adhering to current clinical guidance.

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.

Contribute to practice targets (QOF etc.), complying with local and regional guidance.

Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.).

Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

Support the Clinical Team with all safeguarding matters, in accordance with local and national policies.

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

Deliver opportunistic health promotion where appropriate.
