Job summary
Based in the centre of
Exeter, St Leonards Practice is a CQC outstanding rated GP practice. The
practice is very forward thinking and as well as proving excellent patient care
has a well renowned research team. The role will primarily form part of the
Duty Urgent Care Team. The Team deals with on the day Urgent queries, this role
is for the confident all-rounder who will enjoy the challenge of variety of
physical and mental health presentations in acute primary care. The pay is
competitive and dependent on experience and qualification and includes sick
pay, holiday and NHS pension. There is free onsite parking and we are only a
short 5 minute walk away from high street.
We are looking for an experienced clinician, confident in autonomous practice
with qualification and or educated to the following (or equivalent):
Autonomous RN/NMC practitioner: ideally with prescribing qualification
GPHC Pharmacist: ideally with clinical assessment skills
HCPC Paramedic: ideally with prescribing qualification
Physician associate or other similar transferable qualification
Main duties of the job
Main duties to include:
You should be able to consult with adults and children for most of the medical needs presenting in General Practice including physical and mental health. We conduct a large number of consultations via telephone so experience in this would be preferable.

Pay:Depending on experience.
Hours: 3-4 days a week but can be negotiated.

We are a friendly and supportive team with seven part-time Partners, Nurse Practitioner, management and senior admin team, Practice Nurses, HCAs, admin/reception team. We are a member of the Exeter City Primary Care Network. Our current patient list is approx 9,500.

- To deliver a high standard
of patient care as an Advanced Clinical Practitioner in general practice, using
advanced clinical skills, and a broad and in-depth theoretical knowledge base
- To appropriately triage
assess, examine, investigate, diagnose and treat patients, resulting in the
safe management and/or appropriate referral, following practice guidelines and
protocols
- Order/ interpret
investigations, laboratory tests, x-rays and act on results as appropriate
- The practitioner will need
to prioritise and triage the needs of patients accordingly making any necessary
referrals for investigations in the appropriate manner.
- To manage known
exacerbations of chronic illness ensuring appropriate assessments, treatment
and where appropriate onward referral to appropriate specialities facilitating
self- care and/or care within the community setting.
- The post holder will work
closely with the inter-professional team to support new ways of working that
emphasises a more efficient and patient focused service acting as a
professional clinical role model
- To play an active role in
the development/ improvement and audit of processes that facilitates and
improves patient flow.
- Practice with confidence
and competence within appropriate nationally agreed professional guidelines
such as NMC/ RCN / HEE Guidelines, the Health and Care Professions Council (HCPC)
and the College of Paramedics (COP)
- To ensure that accurate
documentation and records of patient care are kept.
- To provide specialist
advice on patient care to the Practice team taking into account all aspects of
clinical governance, and act as a specialist resource within the Practice
