Job summary

Based in the centre of Exeter, St Leonards Practice is a CQC outstanding rated GP practice. The practice is very forward thinking and as well as proving excellent patient care has a well renowned research team. The role will primarily form part of the Duty Urgent Care Team. The Team deals with on the day Urgent queries, this role is for the confident all-rounder who will enjoy the challenge of variety of physical and mental health presentations in acute primary care. The pay is competitive and dependent on experience and qualification and includes sick pay, holiday and NHS pension. There is free onsite parking and we are only a short 5 minute walk away from high street.

We are looking for an experienced clinician, confident in autonomous practice with qualification and or educated to the following (or equivalent):

Autonomous RN/NMC practitioner: ideally with prescribing qualification GPHC Pharmacist: ideally with clinical assessment skills HCPC Paramedic: ideally with prescribing qualification Physician associate or other similar transferable qualification

Main duties of the job

Main duties to include:

You should be able to consult with adults and children for most of the medical needs presenting in General Practice including physical and mental health. We conduct a large number of consultations via telephone so experience in this would be preferable.

Pay:Depending on experience.

Hours: 3-4 days a week but can be negotiated.

About us

We are a friendly and supportive team with seven part-time Partners, Nurse Practitioner, management and senior admin team, Practice Nurses, HCAs, admin/reception team. We are a member of the Exeter City Primary Care Network. Our current patient list is approx 9,500.

Job responsibilities

If successful you will work within the Duty Care Team to meet the needs of our patients of all ages. We are looking for someone with the right clinical experience and attitude to fill this role. You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, with a commitment to positively contributing to our provision of high-quality patient care. You should possess the skills to work autonomously and have the ability to manage the on the day demand for acute or acute on chronic illnesses.

Previous work experience enabling competent history taking, physical examination, problem solving, and clinical decision making is essential. Preferably you should have a level 7 MSc qualification +/- prescribing or have evidence or willingness of working towards the award.

Key Responsibilities:

  1. To deliver a high standard of patient care as an Advanced Clinical Practitioner in general practice, using advanced clinical skills, and a broad and in-depth theoretical knowledge base
  2. To appropriately triage assess, examine, investigate, diagnose and treat patients, resulting in the safe management and/or appropriate referral, following practice guidelines and protocols
  3. Order/ interpret investigations, laboratory tests, x-rays and act on results as appropriate
  4. The practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
  5. To manage known exacerbations of chronic illness ensuring appropriate assessments, treatment and where appropriate onward referral to appropriate specialities facilitating self- care and/or care within the community setting.
  6. The post holder will work closely with the inter-professional team to support new ways of working that emphasises a more efficient and patient focused service acting as a professional clinical role model
  7. To play an active role in the development/ improvement and audit of processes that facilitates and improves patient flow.
  8. Practice with confidence and competence within appropriate nationally agreed professional guidelines such as NMC/ RCN / HEE Guidelines, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP)
  9. To ensure that accurate documentation and records of patient care are kept.
  10. To provide specialist advice on patient care to the Practice team taking into account all aspects of clinical governance, and act as a specialist resource within the Practice
  11. To enhance the clinical services offered by the practice
  12. To be responsible for your own continuing self-development, undertaking training as appropriate.
  13. To undertake other duties appropriate to the grading of the post as required.
